Our e-forms are designed and provided for the convenience of our clients. Unfortunately, differences in operating systems, printers, drivers, software versions, and many other factors can cause a program to function improperly or not at all. If you have difficulty downloading, editing, or printing our forms, use the following suggestions to help. If the problem persists, please call us, and we will be glad to mail or fax the forms you need.

 

Q: Which version of Adobe Reader do I need to open the downloaded files?

A: The files can be opened with Adobe Reader version 5.0 or later.

 

Q: Why can't I download Adobe Reader from the website?

A: Some antivirus programs can prevent the download of certain programs. Disable you antivirus software and attempt the download again.

 

Q: Why can't I save the completed form on my computer?

A: The Adobe Reader program only allows editing and printing the form. You must purchase Adobe Acrobat software to save the edited form. If you have a scanner, you may scan the printed form and e-mail it to us.

 

Q: Why are the letters and numbers garbled when I print the completed form?

A: This can actually have multiple causes. It’s likely an uncooperative printer font. Try the following:

  1. Instead of using the print icon, select 'File', then 'Print'.
  2. In the print window, select 'Advanced' and choose 'Print as image'.
  3. Select 'File', 'Print', then 'Adobe PDF' as the printer name. Save and print the new form.
  4. Update or reinstall your printer driver software.

 

Q: Do I need to download and install Adobe Reader if I'm using a Mac?

A: No. Use the pre-installed application, Preview, to open the forms.
